Contents
Per 50/100 mcg accuhaler Salmeterol 50 mcg, fluticasone propionate 100 mcg. Per 50/250 mcg accuhaler Salmeterol 50 mcg, fluticasone propionate 250 mcg. Per 50/500 mcg accuhaler Salmeterol 50 mcg, fluticasone propionate 500 mcg. Per 25/50 mcg evohaler Salmeterol 25 mcg, fluticasone propionate 50 mcg. Per 25/125 mcg evohaler Salmeterol 25 mcg, fluticasone propionate 125 mcg. Per 25/250 mcg evohaler Salmeterol 25 mcg, fluticasone propionate 250 mcg
Indications/Uses
Regular treatment of COPD including chronic bronchitis & emphysema. Accuhaler: Regular treatment of asthma in childn & adults. Evohaler: Regular treatment of reversible obstructive airway disease (ROAD) including asthma in childn & adults.
Dosage/Direction for Use
Accuhaler Asthma Adult & adolescent ≥12 yr 1 inhalation of 50/100, 50/250 or 50/500 mcg bd. Childn ≥4 yr 1 inhalation of 50/100 mcg bd. Corticosteroid-sparing adult & adolescent ≥12 yr Fluticasone propionate 250 mcg bd may be replaced w/ 1 inhalation of 50/100 mcg bd. COPD Adult 1 inhalation of 50/250-50/500 mcg bd. Evohaler ROAD Adult & adolescent ≥12 yr 2 inhalations of 25/50, 25/125 or 25/250 mcg bd. Childn ≥4 yr 2 inhalations of 25/50 mcg bd. COPD Adult 2 inhalations of 25/125-25/250 mcg bd.
Contraindications
Hypersensitivity. Accuhaler: Patients w/ severe milk-protein allergy.
Special Precautions
Discontinue use if paradoxical bronchospams occurs. Not for acute symptomatic relief, unstable or acutely deteriorating asthma. Pneumonia in COPD patients. Active or quiescent pulmonary TB, thyrotoxicosis, preexisting CV disease, patients predisposed to low serum K levels. Cushing's syndrome, Cushingoid features, adrenal suppression, decreased bone mineral density, cataract, glaucoma, central serous chorioretinopathy. History of DM. Regularly monitor adrenocortical function in patients transferring form oral steroid therapy. Avoid abrupt w/drawal. High-dose & long-term use. Avoid concomitant use w/ ritonavir. Concomitant use w/ strong CYP3A4 inhibitors eg, ketoconazole. Pregnancy & lactation. Growth retardation in childn & adolescents. Childn <4 yr.
Adverse Reactions
Headache. Mouth & throat candidiasis, pneumonia (COPD patients); hoarseness/dysphonia; muscle cramps, arthralgia.
Drug Interactions
Increased systemic exposure w/ CYP3A4 inhibitors eg, ketoconazole. Avoid non-selective & selective β-blockers.
