Indications/Uses
Dosage/Direction for Use
Major depressive disorder 50 mg once daily. Max: 200 mg daily. Severe renal impairment (CrCl <30 mL/min) or ESRD 50 mg every other day.
Administration
May be taken with or without food: Take at the same time each day. Swallow whole, do not divide/crush/chew/dissolve tab.
Contraindications
Hypersensitivity to desvenlafaxine succinate, venlafaxine HCl. Concomitant use or w/in 14 days of discontinuing treatment w/ MAOI. Patients being treated w/ reversible MAOI eg, linezolid or in whom IV methylene blue has been administered.
Special Precautions
Monitor patients for clinical worsening, suicidality or unusual change in behaviour. Serotonin-syndrome or NMS-like reactions. History of or family history of mania/hypomania; seizure disorders. Patients w/ raised IOP or at risk of acute narrow-angle glaucoma; underlying conditions that may be compromised by increased BP; CV, cerebrovascular or lipid metabolism disorders; increased risk of bleeding; hyponatremia. Control pre-existing HTN before treatment. May cause symptoms of sexual dysfunction. Regularly monitor BP & measure serum lipids during treatment. Avoid abrupt discontinuation. Not to be used concomitantly w/ venlafaxine HCl or other desvenlafaxine succinate-containing drugs. Not recommended to used concomitantly w/ serotonin precursors eg, tryptophan supplements. Concomitant use of aspirin, NSAIDs, warfarin, other anticoagulants; diuretics. May affect ability to drive & use machines. Pregnancy & lactation. Ped <18 yr. Elderly.
Adverse Reactions
Insomnia; headache, dizziness, somnolence; nausea, dry mouth, constipation. Decreased appetite; w/drawal syndrome, anxiety, nervousness, abnormal dreams, irritability, decreased libido, anorgasmia; tremor, paraesthesia, attention disturbance, dysgeusia; blurred vision, mydriasis; vertigo, tinnitus; tachycardia, palpitations; increased BP, hot flush; yawning; diarrhoea, vomiting; hyperhidrosis, rash; musculoskeletal stiffness; erectile dysfunction, delayed ejaculation; fatigue, asthenia, chills, feeling jittery; abnormal LFT, increased/decreased wt.
Drug Interactions
Serotonin syndrome w/ other agents affecting serotonergic neurotransmitter system including triptans, SSRIs, other SNRIs, amphetamines, lithium, sibutramine, fentanyl & its analogues, tramadol, dextromethorphan, tapentadol, meperidine, methadone, pentazocine or St. John's wort; drugs impairing serotonin eg, MAOIs including linezolid, methylene blue; serotonin precursors eg, tryptophan supplements. Increased exposure w/ potent CYP3A4 inhibitors. Increased conc w/ CYP2D6-metabolized drugs. Decreased exposure w/ CYP3A4-metabolized drugs. CNS-active drugs, alcohol. False +ve urine immunoassay screening tests for phencyclidine & amphetamine.
