Sedation of initially intubated & mechanically ventilated patients in ICU. Sedation of non-intubated patients prior to &/or during surgical & other procedures.
ICU sedation Adult Loading dose: 1 mcg/kg IV infusion over 10 min. Maintenance infusion: 0.2-0.7 mcg/kg/hr.Elderly >65 yr & hepatic or renal impairment Dose reduction should be considered. Procedural sedation Loading dose: 1 mcg/kg IV infusion over 10 min. Maintenance infusion: 0.6 mcg/kg/hr, then titrated to 0.2-1 mcg/kg/hr. Less invasive procedures eg, ophth surgery Loading dose: 0.5 mcg/kg IV infusion over 10 min. Maintenance infusion: Initially 0.6 mcg/kg/hr, titrated to 0.2-1 mcg/kg/hr. Awake fiberoptic intubation patient Loading dose: 1 mcg/kg IV infusion over 10 min. Maintenance infusion: 0.7 mcg/kg/hr.
Not to be used as bolus or rapid IV inj. Discontinue use if drug-related hyperthermia or pyrexia is suspected. Significant CV dysfunction, patients w/ advanced heart block &/or severe ventricular dysfunction. Hypotension &/or bradycardia in patients w/ hypovolemia, DM, chronic HTN. Transient HTN during loading dose. W/drawal symptoms. Tolerance & tachyphylaxis if used >24 hr. Not to be co-administered through same IV catheter w/ blood, serum or plasma. Co-administration w/ other vasodilators or -ve chronotropic agents; amphotericin B, diazepam. Concomitant use w/ other anesth, sedatives, hypnotics or opioids. Renal or hepatic impairment. Pregnancy & lactation. Ped. Elderly.
