Indications/Uses
Metabolic acidosis & renal calculi caused by uric acid.
Dosage/Direction for Use
Adult & paed Systemic alkaliser in cardiac arrest Initially 1 mEq/kg then 0.5 mEq/kg IV, may be repeated every 10 min of continued arrest. Less urgent forms of metabolic acidosis; urinary alkaliser 2-5 mEq/kg IV infused over 4-8 hr.
Contraindications
Metabolic or resp alkalosis; Cl loss due to vomiting or continuous GI suction & hypocalcaemia.
Special Precautions
Ensure adequate alveolar ventilation during cardiac arrest to control arterial pH.
Adverse Reactions
Stomach belching or swelling due to CO2 release, stomach cramps, unusual increased thirst.
Drug Interactions
Adrenocorticoids, mineralocorticoids, corticotropin, amphetamines, quinidine, androgens, anabolic steroids, antimuscarinics, ciprofloxacin, K-depleting diuretics (eg, bumetanide, thiazide, ethacrynic acid, furosemide, indapamide), mecamylamine, methenamine, mexiletine, K supplements, salicylates.
