Indications/Uses
Adjunctive therapy in treatment of partial seizures w/ & w/o secondary generalization in adults & childn ≥3 yr. Neuropathic pain including diabetic neuropathy, post-herpetic & trigeminal neuralgia in adults ≥18 yr.
Dosage/Direction for Use
Epilepsy Adult & childn >12 yr 900-3,600 mg daily. Initially 300 mg tds on day 1 or by titrating dose as: 300 mg once daily on day 1, 300 mg bd on day 2, 300 mg tds on day 3. Thereafter, dose may be increased in 3 equally divided doses up to max: 3,600 mg daily. Childn 3-12 yr Initially 10-15 mg/kg tds, ≥5 yr 25-35 mg/kg tds. 3 to <5 yr 40 mg/kg tds. Neuropathic pain Adult 900 mg daily in 3 equally divided doses, increase if necessary up to max: 3,600 mg daily. Renal impairment in patient w/ neuropathic pain or epilepsy CrCl ≥80 mL/min 900-3,600 mg daily, 50-79 mL/min 600-1,800 mg daily, 30-49 mL/min 300-900 mg daily, 15-29 mL/min 150-600 mg daily, <15 mL/min 150-300 mg daily. Patient undergoing hemodialysis Loading dose: 300-400 mg then 200-300 mg following each 4-hr hemodialysis.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Discontinue treatment if signs or symptoms of anaphylaxis occurs. Not effective for absence seizures. Drug rash w/ eosinophilia & systemic symptoms. Increased risk of suicidal thoughts or behavior. Patients w/ compromised resp function, resp or neurological disease, dizziness, somnolence. Monitor for drug abuse & dependence. Avoid abrupt w/drawal. Concomitant use w/ opioids; CNS depressants. May affect ability to drive or operate machinery. Renal impairment. Pregnancy & lactation. Childn <3 yr w/ epilepsy & <18 yr w/ neuropathic pain.
Adverse Reactions
Fatigue, headache; nausea &/or vomiting; ataxia, dizziness, nystagmus, somnolence, tremor; diplopia.
Drug Interactions
Increased AUC w/ morphine. Reduced bioavailability w/ Al- & Mg-containing antacids. False +ve readings w/ Ames N-Multistix SG dipstick test when added to other anti-convulsants.
