Phenylephrine HCl
Indications/Uses
Pupillary dilation in uveitis, refraction w/o cycloplegic. Funduscopy & other diagnostic procedures.
Dosage/Direction for Use
Vasoconstriction & pupil dilation Instill 1 drop after few min of suitable topical anesth. Posterior synechiae uveitis Instill 1 drop, may be repeated if necessary. Max: Not to exceed 3 times. Glaucoma Instill 1 drop w/ miotics. Surgery Instill 1 drop 30-60 min pre-op. Refraction Adult Instill 1 drop 5 min after preferred cycloplegic. Ophthalmoscopic exam Instill 1 drop in each eye. Blanching test Instill 1 or 2 drops; examine after 5 min.
Contraindications
Hypersensitivity. Anatomically narrow angles or narrow-angle glaucoma. Newborn, infants & elderly w/ severe arteriosclerotic, CV or cerebrovascular disease.
Special Precautions
Not for intraocular use. Patients w/ sympathetic denervation eg, insulin-dependent diabetes, orthostatic hypotension, HTN, hyperthyroidism; instrumented, traumatised, diseased or post-op eye or adnexa; suppressed lacrimation during anesth; rebound miosis. Dilation prior to retinal detachment or cataract surgery. Remove contact lens prior to application & wait 15 min before reinsertion. Avoid contact w/ container tip. High doses. May cause eye irritation & soft contact lens discoloration due to benzalkonium Cl. Concomitant use w/ or up to 21 days after MAOI therapy; TCAs, β-blockers eg, propanolol; atropine. May affect ability to drive & use machines. Pregnancy & lactation. Not recommended in preterm & newborn infants. Childn. Elderly.
Adverse Reactions
Hypersensitivity; dizziness; eye pain & irritation, ocular hyperaemia, conjunctivitis; increased BP, tachycardia; pulmonary oedema; contact dermatitis.
Drug Interactions
Exaggerated adrenergic effects w/ MAOIs. Potentiated pressor response by TCAs, certain antihypertensives eg, guanethidine, reserpine, non-selective β-blockers eg, propranolol. Enhanced pressor effects & induced tachycardia w/ atropine. Potentiated CV depressant effects of potent inhalation anesth agents.
