Indications/Uses
Adjunct to general anesth to relax skeletal muscles, facilitate tracheal intubation & mechanical ventilation.
Dosage/Direction for Use
IV inj Tracheal intubation Adult 0.2 mg/kg over 30 sec or 0.25 mg/kg as divided dose (0.15 mg/kg followed by 0.1 mg/kg after 30 sec). Bolus dose: 0.07-0.25 mg/kg. Childn 7 mth-12 yr & infant 0.2 mg/kg. Bolus dose: 0.1-0.2 mg/kg over 5-15 sec. Maintenance: 0.1 mg/kg. Infant 2-6 mth Bolus dose: 0.1-0.15 mg/kg over 5-15 sec. Maintenance: 0.1 mg/kg. IV infusion Adult 8-10 mcg/kg/min. Dose adjustment may be given in increments approx 1 mcg/kg/min. Childn 2-12 yr Approx 13-14 mcg/kg/min (range: 5-31 mcg/kg/min) under halothane or narcotic anaesth, 7-23 mth Approx 11 mcg/kg/min (range: 3-26 mcg/kg/min). Infant 2-6 mth Approx 11 mcg/kg/min (range: 4-24 mcg/kg/min).
Contraindications
Hypersensitivity. Patients known to be homozygous for atypical plasma cholinesterase gene.
Special Precautions
Hypersensitivity to other neuromuscular blocking agents. Increased sensitivity to effects of histamine. Genetically determined abnormalities of plasma cholinesterase; severe generalized tetanus, TB & other severe or chronic infections; chronic debilitating disease, malignancy, chronic anaemia, malnutrition; myxoedema, collagen diseases; decompensated heart disease; peptic ulcer; burn patients; iatrogenic. Severe acid-base or electrolyte abnormalities; myasthenia gravis, other forms of neuromuscular disease, cachectic patients; hypovolaemic; malignant hyperthermia-susceptible patients. Reversal of neuromuscular block. Not to be mixed w/ highly alkaline soln. May affect ability to drive & use machines. End-stage hepatic failure; acute, chronic or end-stage renal failure. Not to be used during pregnancy. Lactation. Elderly.
Drug Interactions
Potentiated by inhalational anaesth eg, enflurane, isoflurane, sevoflurane, halothane. Increased magnitude &/or duration w/ aminoglycosides, polymyxins, spectinomycin, tetracyclines, lincomycin, clindamycin; propranolol, Ca-channel blockers, lidocaine, procainamide, quinidine; furosemide, possibly thiazides, mannitol, acetazolamide; Mg & lithium salts; ketamine; trimetaphan, hexamethonium. Prolonged neuromuscular-blocking action w/ drugs reducing plasma cholinesterase activity eg, anti-mitotic drugs, MAOIs, ecothiopate iodide, pancuronium, organophosphates, anticholinesterases, certain hormones, bambuterol, SSRI. Aggravated or unmasked latent myasthenia gravis w/ β-blockers, anti-arrhythmics, antirheumatics, trimetaphan, chlorpromazine, steroids, phenytoin, lithium. Prolonged & complex block w/ suxamethonium Cl.
