Indications/Uses
Neuropathic pain in adults. Adjunctive therapy in adults w/ partial seizures w/ or w/o secondary generalisation. Generalised anxiety disorder (GAD) in adults, management of fibromyalgia.
Dosage/Direction for Use
Neuropathic pain Initially 150 mg daily in 2 or 3 divided doses, may be increased to 300 mg daily after 3-7 days interval. Max: 600 mg daily after an additional 7-day interval. Epilepsy Initially 150 mg daily, may be increased to 300 mg daily after 1 wk. Max: 600 mg daily after an additional wk. GAD Initially 150 mg daily, may be increased to 300 mg daily after 1 wk, then to 450 mg daily following an additional wk. Max: 600 mg daily after an additional wk. Fibromyalgia 300-450 mg daily in 2 divided doses; start at 75 mg bd & may be increased to 150 mg bd w/in 1 wk or may be further increased to 225 mg bd. Max: 600 mg daily after an additional wk.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Discontinue treatment if hypersensitivity; life-threatening angioedema; diagnosed or suspected myopathy or markedly elevated creatine kinase levels occur. Increased risk of suicidal thoughts or behavior. Peripheral edema; CHF patients w/ NYHA class III or IV; dizziness & somnolence; loss of consciousness, confusion & mental impairment; wt gain; prolonged PR interval; encephalopathy; tumorigenic potential; blurred vision. Evaluate patients for history of drug abuse & signs of misuse or abuse or dependence eg, development of tolerance, dose escalation, drug-seeking behavior. Avoid abrupt or rapid discontinuation. Taper-off gradually over at least 1 wk. Concomitant use w/ anti-spasticity agents. Not to be taken by patients w/ galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. May affect ability to drive & use machines. Women of childbearing potential must use effective contraception. Pregnancy. Not recommended during lactation. Childn <12 yr & adolescents. Elderly >65 yr.
Adverse Reactions
Dizziness, somnolence. Nasopharyngitis; increased appetite; confusion, disorientation, irritability, depression, euphoric mood, decreased libido, insomnia, ataxia, abnormal coordination, balance disorder, amnesia, disturbance inattention, memory impairment, tremor, dysarthria, paresthesia, hypoesthesia, sedation, lethargy, blurred, diplopia, vertigo, vomiting, abdominal distension, constipation, dry mouth, flatulence, muscle cramp, arthralgia, back & limb pain, cervical spasm, peripheral oedema, oedema, abnormal gait, fall, feeling drunk & abnormal, fatigue, increased wt.
Drug Interactions
Potentiated ethanol & lorazepam effects. Additive impairment of cognitive & gross motor function w/ oxycodone. Resp failure & coma w/ other CNS depressants. Reduced lower GI tract function w/ opioid analgesics.
