Lidocaine HCl
Indications/Uses
Most major nerve blocks eg, sciatic & retrobulbar.
Dosage/Direction for Use
Adult & childn >12 yr Retrobulbar nerve block 80 mg. Sciatic nerve block 300-400 mg. Max: 400 mg.
Contraindications
Hypersensitivity to amide-type local anaesth, methylparaben, its metabolite or PABA. Inflammation &/or sepsis in the inj region &/or septicaemia. Stokes-Adams syndrome, Wolff-Parkinson-White syndrome, severe degrees of SA, AV or intraventricular block in absence of artificial pacemaker.
Special Precautions
Acute CNS & CV toxic effects. Constantly monitor CV, resp vital signs & consciousness. 2nd or 3rd degree AV block, severe hepatic disease, severe renal impairment, concomitant class III antiarrhythmics (eg, amiodarone). Epilepsy, impaired cardiac conduction, bradycardia, severe shock or digitalis intoxication, impaired CV function. Acute porphyria. May impair ability to drive or operate machinery. Pregnancy & lactation. Elderly, young or debilitated patients.
Adverse Reactions
Nausea, vomiting, hypotension, HTN, bradycardia, cardiac arrest, arrhythmia, paraesthesia, dizziness, CNS toxicity symptoms, neuropathy, peripheral nerve injury, arachnoiditis, allergic reactions, double vision, resp depression.
Drug Interactions
Toxic plasma conc & reduced clearance w/ cimetidine. Additive effect w/ class 1B antiarrhythmics. Potentiation of cardiac effects w/ disopyramide, procainamide, mexiletine. Reduced clearance w/ amiodarone. Reduced metabolism w/ propranolol, metoprolol. Enhanced metabolism w/ phenytoin, phenobarb, primidone, carbamazepine. Decreased min effective conc of nitrous oxide. Excessive neuromuscular blockade w/ suxamethonium.
