Quetiapine fumarate
Indications/Uses
Schizophrenia. Monotherapy or adjunct to lithium or divalproex for acute manic episodes associated w/ bipolar 1 disorder.
Dosage/Direction for Use
Adult Initially 25 mg bd, increased to 25-50 mg bd or tds on 2nd or 3rd day, to target range: 300-400 mg daily by 4th day bd or tds. Range: 150-750 mg daily. Acute manic episodes associated w/ bipolar disorder Total daily dose for 1st 4 days of therapy: 100 mg (day 1), 200 mg (day 2), 300 mg (day 3) & 400 mg (day 4), adjusted up to 800 mg daily by day 6 in no >200 mg daily increments. Usual effective dose range: 400-800 mg daily. Elderly, renal & hepatic impairment Initially 25 mg daily, increased in increments of 25-50 mg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Concomitant use w/ CYP450 3A4 inhibitors eg, HIV PIs, azole antifungal agents, erythromycin, clarithromycin, nefazodone.
Special Precautions
Discontinue use if NMS occurs or signs & symptoms of tardive dyskinesia appears. CV disease, family history of QT prolongation, cerebrovascular disease or other conditions predisposing to hypotension; history of seizures; extrapyramidal symptoms; hyperglycemia, DM; congenital long QT syndrome, CHF, heart hypertrophy, hypokalaemia, hypomagnesaemia. Patients w/ risk factors for stroke. Avoid abrupt w/drawal. Not to be taken by patients w/ galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Avoid use w/ other neuroleptics. Concomitant use w/ strong hepatic enzyme inducer eg, carbamazepine or phenytoin; medicines increasing QTc interval. May affect ability to drive & use machines. Renal & hepatic impairment. Pregnancy. Avoid use during lactation. Childn & adolescents. Elderly w/ dementia-related psychosis.
Adverse Reactions
Dizziness, somnolence, headache. Leucopenia; syncope; tachycardia; orthostatic hypotension; rhinitis; dry mouth, constipation, dyspepsia; mild asthenia, peripheral edema; wt gain, serum transaminases elevation.
Drug Interactions
Increased AUC w/ ketoconazole. Increased clearance w/ phenytoin, thioridazine. Not recommended w/ grapefruit juice. Medicines increasing QTc interval; drugs causing electrolyte imbalance; centrally-acting drugs, alcohol.
