Indications/Uses
Herpes simplex skin & mucous membrane infections including initial & recurrent genital herpes. Suppression, prevention & prophylaxis of recurrent herpes simplex infections in immunocompetent patients. Varicella (chickenpox), herpes zoster (shingles) infections.
Dosage/Direction for Use
Adult Herpes simplex infections 200 mg 5 times daily at 4-hrly intervals for 5 days. Severe immunocompromised patient 400 mg 5 times daily. Severe renal impairment 200 mg bd. Suppression of herpes infections in immunocompetent patient 200 mg qds at 6-hrly intervals or 400 mg bd at 12-hrly intervals. Prophylaxis of herpes simplex infections 200 mg qds at 6-hrly intervals. Severely immunocompromised patient 400 mg qds. Varicella & herpes zoster infections 800 mg 5 times daily at 4-hrly intervals for 7 days. Severe renal impairment (CrCl <10 mL/min) Management of herpes simplex infections 200 mg bd at 12-hrly intervals. Herpes zoster infections 800 mg bd at 12-hrly intervals. Moderate renal impairment (CrCl 10-25 mL/min) Herpes zoster infections 800 mg tds at 8-hrly intervals.
Administration
May be taken with or without food: May be taken w/ meals to reduce GI discomfort.
Contraindications
Hypersensitivity to aciclovir or valaciclovir.
Special Precautions
Ensure adequate hydration. Severe renal impairment. Pregnancy & lactation. Elderly.
Adverse Reactions
Nausea, vomiting, diarrhoea, abdominal pain; rashes including photosensitivity, urticaria; headache.
Drug Interactions
Increased plasma conc w/ drugs eliminated via active renal tubular secretion. Increased AUC by probenecid, cimetidine. Increased plasma AUC of mycophenolate mofetil.
