Indications/Uses
Adjunct to diet & exercise in NIDDM whenever blood sugar levels cannot be controlled by diet, physical exercise & wt reduction alone. May be used in combination w/ metformin or insulin.
Dosage/Direction for Use
Initially 1 mg once daily, may be increased gradually at intervals of 1-2 wk in a stepwise manner: 1 mg-2 mg-3 mg-4 mg-6 mg. Max: 8 mg daily. Patient w/ well-controlled diabetes 1-4 mg once daily.
Administration
Should be taken with food: Take immediately before the 1st main meal of the day. Do not skip meals.
Contraindications
Hypersensitivity to glimepiride, other sulphonylureas & sulfonamides. Porphyria. Severe hepatic & renal impairment. Pregnancy & lactation.
Special Precautions
Increased risk of CV mortality. Regularly monitor glucose levels in blood & urine. Milder or absent symptoms of hypoglycaemia in elderly, in patients w/ autonomic neuropathy or those receiving concurrent treatment w/ β-blockers, clonidine, reserpine, guanethidine, other sympatholytics. May affect ability to drive or operate machinery.
Adverse Reactions
Hypoglycaemia; temporary visual impairment; blood changes; allergic or pseudoallergic reactions.
Drug Interactions
Potentiated blood glucose-lowering effect w/ insulin & other oral antidiabetics, MAOIs, ACE inhibitors, miconazole, anabolic steroids & male sex hormones, para-aminosalicylic acid, chloramphenicol, high-dose parenteral pentoxifylline, coumarin derivatives, phenylbutazone, azapropazone, oxyphenbutazone, cyclophosphamide, probenecid, disopyramide, quinolones, fenfluramine, salicylates, fenyramidol, sulphinpyrazone, fibrates, sulfonamides, fluoxetine, tetracyclines, guanethidine, tritoqualine, ifosfamide, trofosfamide. Weakened blood glucose-lowering effect w/ acetazolamide, laxatives (after protracted use), barbiturates, high-dose nicotinic acid, corticosteroids, oestrogens & progestogens, diazoxide, phenothiazines, diuretics, phenytoin, epinephrine & other sympathomimetic agents, rifampicin, glucagons, thyroid hormones. Potentiated or weakened blood glucose-lowering effect w/ H2-receptor antagonists, β-blockers, clonidine, guanethidine, reserpine, acute & chronic alcohol intake. Potentiated or weakened effects of coumarin derivatives.
