Depression, obsessive-compulsive disorder, premenstrual dysphoric disorder (PMDD).

Dosage/Direction for Use

Adult & elderly Depression w/ or w/o anxiety 20 mg daily. Obsessive-compulsive disorder 20-60 mg daily. PMDD 20 mg daily for 6 mth.

Overdosage

Administration

May be taken with or without food.

Contraindications

Hypersensitivity. Concomitant use w/ MAOIs.

Special Precautions

Discontinue use if rash & allergic reactions occur. History of seizures, unstable seizure disorders/epilepsy, controlled epilepsy; history of mania/hypomania; acute cardiac disease; diabetes; wt loss; history of bleeding disorder. Suicidal thoughts & behaviour. Concomitant use w/ oral anticoagulants, atypical antipsychotics, TCAs, aspirin, NSAIDs, St. John's wort, drugs increasing risk of bleeding & those receiving electroconvulsive (ECT) treatment. May affect ability to drive & use machines. Hepatic & renal dysfunction. Pregnancy & lactation. Suicidal thoughts in childn & adolescents.

Adverse Reactions

Hypersensitivity, chills, serotonin syndrome, photosensitivity; GI disorders, dry mouth; headache, sleep abnormalities, dizziness, anorexia, fatigue, euphoria, transient abnormal movement, seizures & psychomotor restlessness, hallucinations, manic reaction, confusion, agitation, anxiety, depersonalisation, panic attacks; urinary retention & frequency; sexual dysfunction, priapism, galactorrhoea; alopecia, yawn, abnormal vision, sweating, vasodilatation, arthralgia, myalgia, postural hypotension, ecchymosis; pharyngitis, dyspnoea.

Drug Interactions

Risk of serotonin syndrome w/ MAOI-B (eg, selegiline), serotonergic drugs, lithium or tryptophan. Changed blood levels w/ phenytoin. Altered anticoagulant effects. Increased adverse effects w/ St. John's wort. Prolonged seizures w/ ECT. Alcohol. Drugs w/ narrow therapeutic index eg, flecainide, encainide, carbamazepine, TCAs.
