Indications/Uses
Mild to moderately severe dementia associated w/ Alzheimer's or Parkinson's disease & severe dementia associated w/ Alzheimer's disease.
Dosage/Direction for Use
Mild to moderately severe dementia associated w/ Alzheimer's or Parkinson's disease Start w/ Exelon Patch 5 once daily, may be increased to Exelon Patch 10 after min of 4 wk. Severe dementia associated w/ Alzheimer's disease Start w/ Exelon Patch 5 once daily, may be increased to Exelon Patch 10 & then to Exelon Patch 15.
Contraindications
Hypersensitivity to rivastigmine, other carbamate derivatives. History of application site reactions suggestive of allergic contact dermatitis w/ rivastigmine transdermal patch.
Special Precautions
Discontinue use if disseminated allergic dermatitis occurs. Application site reactions & skin reactions. GI disorders, risk of dehydration in case of prolonged vomiting or diarrhoea; sick sinus syndrome or conduction defects eg, SA & AV block; active gastric or duodenal ulcers, patients w/ predisposing factors; patients predisposed to urinary obstruction & seizures; history of asthma or obstructive pulmonary disease. ECG QT prolongation; patients w/ uncompensated heart failure, recent MI, bradyarrhythmias, hypokalemia or hypomagnesemia, personal or family history of QT prolongation, or concomitant use w/ medicinal products inducing QT prolongation &/or Torsade de pointes. May induce or exacerbate extrapyramidal symptoms. Medication misuse & dosing errors resulting in overdose. Monitor patient's wt during therapy. Not to be applied to red, irritated or cut skin. Avoid exposure to any external heat sources eg, excessive sunlight, saunas, solarium. Patients <50 kg. May affect ability to drive & use machines. Hepatic impairment. Pregnancy. Not to be used during lactation. Not recommended in ped patients <18 yr.
Adverse Reactions
Nausea. Anorexia, decreased appetite; anxiety, depression, insomnia; dizziness, headache; vomiting, diarrhoea, dyspepsia, abdominal pain; urinary incontinence; application site reactions, erythema, pruritus & oedema, fatigue, asthenia; decreased wt; UTI.
Drug Interactions
Additive extrapyramidal effect w/ metoclopramide. Possible additive effect w/ other cholinomimetic drugs. Interfered activity of anticholinergic medications eg, oxybutynin, tolterodine. Exaggerated effects of succinylcholine-type muscle relaxants during anaesth. Additive effects leading to bradycardia w/ β-blockers. Increased oral clearance by nicotine. QT interval-prolonging drugs eg, quinidine, amiodarone, pimozide, halofantrine, cisapride, citalopram, mizolastin, moxifloxacin, erythromycin.
