Erythromycin lactobionate
Indications/Uses
Upper & lower resp tract infections; skin & soft tissue & other infections. Adjunct to antitoxin, syphilis & gonorrhoea, subacute bacterial endocarditis, otitis media.
Dosage/Direction for Use
Adult Usual dose: 25-50 mg/kg daily in divided doses by IV infusion every 6 hr or 1-2 g daily by intermittent IV infusion over 20-60 min every 6 hr or by infusion over 24 hr. Severe infections 4 g daily. Childn 25-50 mg/kg daily in divided doses by IV inj.
Contraindications
Hypersensitivity. Not to be administered rapidly by direct IV inj (IV push). Prolonged QT interval & development of ventricular arrhythmias. Concomitant use w/ simvastatin or lovastatin, astemizole, terfenadine, cisapride or pimozide.
Special Precautions
Discontinue use if severe acute hypersensitivity reactions occur. Myasthenia gravis. Superinfection; overgrowth of nonsusceptible organisms due to prolonged use. Concomitant use w/ lovastatin, simvastatin or other statins. Hepatic impairment. Severe renal impairment. Not recommended during pregnancy. Lactation. Infantile hypertrophic pyloric stenosis. Elderly.
Adverse Reactions
Cholestatic hepatitis, jaundice; GI disturbances; thrombophlebitis, venous irritation, prolonged QT interval, ventricular arrhythmias; transient perceptive deafness (very high doses); mild allergic reactions, urticaria, skin eruptions, rashes w/ fever, erythema multiforme.
Drug Interactions
Inhibited bactericidal effect of penicillin. Potentiated effects w/ digoxin, warfarin, carbamazepine, high-dose theophylline. Myalgia, neutropenia & fever w/ vinblastine. Increased serum conc of alfentanil, bromocriptine, hexobarbitone, midazolam, phenytoin, tacrolimus, triazolam, valproate, acenocoumarol. Increased serum levels of simvastatin, lovastatin or other statins. Increased plasma levels of cyclosporin. Ergotism w/ ergot. Increased risk of cardiotoxicity w/ astemizole, terfenadine, cisapride, pimozide. Increased toxicity w/ theophylline. Cardiac toxicity or Torsade de pointes w/ drugs prolonging QT interval. Interfered w/ serum folate measurements. Verapamil, clozapine.
