Per g Lidocaine 25 mg, prilocaine 25 mg
Indications/Uses
Surface anaesth of skin during needle insertion, superficial surgical procedures; leg ulcers prior to cleaning & superficial surgical procedures eg, fibrin, pus & necroses removal; genital mucosa.
Dosage/Direction for Use
Apply thick layer w/ occlusive dressing. Adult Needle insertion eg, IV cannula insertion, taking of blood samples Approx 2 g/10 cm2 for 1 hr. Max: 5 hr. Minor superficial surgical procedures eg, warts removal 1.5-2 g/10 cm2 for 1 hr. Max: 5 hr. More extensive superficial surgical procedures in hospital setting eg, split skin grafting 1.5-2 g/10 cm2 for 2 hr. Max: 5 hr. Cleaning of leg ulcers Approx 1-2 g/10 cm2 for 30 min, may extend to 60 min for leg ulcers w/ tissue. Max: Not >10 g/treatment occassion. Genital mucosa Men 1 g/10 cm2 for 15 min. Women 1-2 g/10 cm2 for 60 min. Removal of genital warts (condylomata acuminata) & prior to local anaesth inj Approx 5-10 g for 5-10 min. No occlusive dressing required. Childn Needle insertion, removal of mollusca & other minor superficial surgical procedures Max: 1 g/10 cm2. Childn 6-12 yr Max: 20 g/200 cm2 for 1 hr up to 5 hr, 1-6 yr Max: 10 g/100 cm2 for 1 hr up to 5 hr, 3-12 mth Max: 2 g/20 cm2 for 1 hr, 0-3 mth Max: 1 g/10 cm2 not >1 hr. Atopic dermatitis Reduced application time: 30 min.
Contraindications
Hypersensitivity to amide-type anaesth.
Special Precautions
Not to be applied to open wounds other than leg ulcers, genital mucosa of childn, impaired tympanic membrane. G6PD deficiency or congenital or idiopathic methaemoglobinaemia; atopic dermatitis. Avoid contact w/ eyes. Monitor patients treated w/ class III antiarrhythmic drugs (eg, amiodarone) & live vaccines. Pregnancy. Increased in methaemoglobin level in childn/neonates <3 mth. Not to be used in infants 0-12 mth receiving methaemoglobin-inducing agents, preterm infants w/ gestational age <37 wk & heel lancing in neonates.
Adverse Reactions
Paleness, erythema (redness), oedema. Mild burning sensation, itch or warmth in leg ulcer & genital mucosa.
Drug Interactions
Increased risk of additive systemic toxicity w/ other local anaesth. Increased methaemoglobin level w/ suphonamides. Reduced lidocaine clearance w/ cimetidine or β-blockers.
