Indications/Uses
Management of chronic pain & intractable pain in patients requiring continuous opioid administration for an extended period of time.
Dosage/Direction for Use
Individualised dosage based on patient's status. Lowest effective dose should be used.
Contraindications
Hypersensitivity. Not to be used in acute or post-op pain management. Patients w/ significant resp depression.
Special Precautions
Patients who experienced serious adverse events should be monitored for at least 24 hr after patch removal. Do not switch from 1 transdermal patch system to another w/o proper evaluation & supervision. Do not cut the patches. Careful observation for potential resp depression especially in opioid-naive & not opioid-tolerant patients. Central sleep apnea & sleep-related hypoxia; chronic pulmonary disease; tolerance, physical & psychological dependence may develop upon repeated administration of opioids; CNS conditions including increased intracranial pressure; patients w/ bradyarrhythmia; adrenal insufficiency; decreased sex hormone levels & symptoms (low libido, erectile dysfunction or infertility); opioid-induced hyperalgesia; consider advising on measures to prevent constipation & prophylactic laxative; hepatic & renal impairment. Avoid exposure of application site to direct external heat sources. Serotonin syndrome w/ concomitant use of serotonergic drugs. May impair ability to drive & use machines. May cause reduced fertility in females & males of reproductive potential. Not to be used during pregnancy. Not recommended during lactation. Childn <2 yr. Elderly patients.
Adverse Reactions
Anorexia; insomnia, depression; somnolence, dizziness; vertigo; palpitations; nausea, vomiting, constipation, upper abdominal pain, dry mouth; hyperhidrosis, pruritus, rash; muscles spasms; fatigue, feeling cold, malaise, asthenia, peripheral edema; hypersensitivity; anxiety, confusional state, hallucination; headache, tremor, paresthesia; diarrhea, abdominal pain; erythema; urinary retention.
Drug Interactions
Disproportionately increased CNS depressant effects w/ CNS depressants (alcohol, benzodiazepines, illegal drugs). Potentiated opiate or serotonergic effects w/ MAOIs. Increased risk of serotonergic syndrome w/ serotonergic drugs. Increased plasma conc w/ CYP3A4 inhibitors. Decreased plasma conc w/ CYP3A4 inducers.
