Propofol

Induction & maintenance of general anaesth, sedation of ventilated adult patients receiving intensive care & conscious sedation for surgical & diagnostic procedures.

Adult Induction: Titrated approx 40 mg every 10 sec by slow bolus inj or infusion, <55 yr 1.5-2.5 mg/kg. Total dose can be reduced to 20-50 mg/min. Patient of ASA grades 3 & 4 Approx 20 mg every 10 sec. Maintenance: 4-12 mg/kg/hr by continuous infusion or in increments of 25-50 mg by repeat bolus inj. Sedation during intensive care 0.3-4 mg/kg/hr by continuous infusion. Sedation for surgical & diagnostic procedures 0.5-1 mg/kg over 1-5 min to initiate sedation. Maintenance: 1.5-4.5 mg/kg/hr, may use 10-20 mg bolus inj for rapid increase in depth of sedation. Childn Induction: Given slowly until clinical signs show onset of anaesth. Dose adjusted for age &/or wt, >8 yr 2.5 mg/kg. Maintenance: 9-15 mg/kg/hr.

Hypersensitivity to propofol, peanut/soya. Sedation in intensive care of patients ≤16 yr.

Patients predisposed to Zn deficiency eg, burns, diarrhoea &/or sepsis; decreased O₂ delivery to tissues; serious neurological injury; ophth surgery; acute pulmonary insufficiency or resp depression; epileptic patients; cardiac or resp impairment; hypovolaemic patients; bradycardia; fat metabolism disorders. Monitor for signs of hypotension, airway obstruction & O₂ desaturation; blood lipid levels regularly. Sexual disinhibition may occur. Avoid alcohol before & at least 8 hr after administration. Not recommended w/ electroconvulsive treatment. Concomitant use w/ CNS depressants eg, potent & narcotic analgesics, general anaesth; high-dose vasoconstrictors, steroids, inotropes. May affect ability to drive & use machines. Renal or hepatic impairment. Not to be used for obstetric anaesth, during pregnancy & lactation. Not for general anaesth in childn <1 mth. Not recommended in childn <3 yr for general anaesth induction & maintenance. Sedation in childn <16 yr. Elderly or debilitated patients.

Local pain on induction. Hypotension; bradycardia; transient apnoea; nausea, vomiting; headache; w/drawal symptoms & flushing in childn.

Additive effects w/ other CNS depressants eg, premed drugs, inhalation & analgesic agents. Concomitant use w/ valproate.
