Pentamidine isethionate
Alternative 1st-line treatment of Pneumocystis carinii infection in AIDS patients & Leishmania aethiopica; 2nd-line treatment of Pneumocystis carinii infection in non-AIDS patients, Leishmaniasis (visceral & cutaneous) & Trypanosomiasis (except for Trypanosomiasis rhodesiense).
Dosage/Direction for Use
Pseudomonas carinii pneumonia 4 mg/kg slow IV infusion once daily for 14 days. Leishmaniasis Visceral: 3-4 mg/kg on alternate days to max: 10 inj. Cutaneous: 3-4 mg/kg once or twice wkly until condition resolves. Trypanosomiasis 4 mg/kg daily or on alternate days to total: 7-10 inj.
Special Precautions
Not approved for inhaled use. Not to be inj IM. Hypersensitivity reactions at inj site eg, skin rash, erythema. Acute pancreatitis, cardiac arrhythmias, hypocalcaemia, ventricular tachycardia; malnutrition, hyper-/hypoglycaemia, HTN/hypotension, anaemia, leucopenia or thrombocytopenia; ECG abnormalities eg, QT interval prolongation, Torsade de pointes; DM; nausea, vomiting; phlebitis. Monitor BUN, serum creatinine, Ca & electrolytes, CBC & platelet counts, fasting blood glucose, LFTs, urine analysis & ECG before, during & after treatment. Avoid extravasation. Acute renal failure; hepatic & renal dysfunction. Not to be used during pregnancy & lactation. Childn. Elderly.
Adverse Reactions
Severe hypotension & hypoglycaemia; acute pancreatitis; cardiac arrhythmias & arrest, ventricular tachycardia; leucopenia, thrombocytopenia; Herxheimer reaction; hypocalcaemia; toxic delirium; syncope; acute renal failure, renal impairment; SJS. Anaemia; hyperglycaemia, hyperkalaemia, hyponatraemia, DM; hallucinations; taste disturbance, dizziness; tachycardia, bradycardia; facial flushing, venous thrombosis; breathlessness; nausea, vomiting; hepatic dysfunction; abscess &/or necrosis, rash, itching, alopecia, erythema multiforme; azotaemia, albuminuria, glycosuria, increased creatinine levels; inj site reactions; depressed serum folate.

Additive nephrotoxic effects w/ other nephrotoxic drugs.
