HTN 4 mg in the morning, may be increased to 8 mg after 1 mth. Elderly 2 mg in the morning, may be increased to 4 mg after 1 mth. CHF 2 mg in the morning, may be increased to 4 mg after 15 days. CrCl between 30 & 60 mL/min 2 mg daily, CrCl between 15 & 30 mL/min 2 mg every other day, CrCl <15 mL/min 2 mg on dialysis day.
Risk of arterial hypotension; renovascular HTN. Patients undergoing major surgery or during anaesth. W/draw diuretic at least 3 days prior to initiation in hypertensive patients previously treated w/ diuretic. Perform plasma creatinine assay prior to & during treatment. Renal failure. Elderly.
Induced hyperkalemia w/ K supplements or K-sparing diuretics. Severe initial fall in BP w/ diuretics. Increased risk of orthostatic hypotension w/ neuroleptic or TCAs.
