Indications/Uses
Skin & soft tissue infections caused by pneumococci; upper & lower resp tract infection.
Dosage/Direction for Use
Adult 250 mg IM every 4-6 hr or 500 mg IV over 3-4 min every 4-6 hr. Childn 2-10 yr ½ adult dose. Infant up to 2 yr ¼ adult dose.
Contraindications
Hypersensitivity to penicillins. Not to be used for infections due to penicillinase-producing organisms.
Special Precautions
Hypersensitivity to penicillins. Not to be given in patients w/ infectious mononucleosis. Risk of Jarisch-Herxheimer in patients w/ syphilis. High-dose use. Not be mixed w/ proteinaceous fluids eg, protein hydrolysates, blood or plasma, or w/ IV lipid emulsions. Severe renal impairment (CrCl <10 mL/min). Pregnancy & lactation.
Adverse Reactions
Nausea, diarrhea, digestive disorders; maculopapular exanthema; haemolytic anemia, neutropenia; convulsion. Jarisch-Herxheimer reactions.
Drug Interactions
Substantial mutual inactivation w/ aminoglycosides. Higher serum peak conc & prolonged serum conc w/ probenecid. Increased risk of skin rash w/ allopurinol. Prolonged &/or worsened diarrhea w/ antiperistaltic antidiarrheals. Interfered bactericidal effects by chloramphenicol, erythromycin, sulfonamides, tetracyclines. Decreased effectiveness of OCs.
