Cefuroxime (tab: axetil; vial: Na)
Indications/Uses
Upper & lower resp tract, GUT, ENT, skin & soft tissue, bone & joint infections, UTI, O&G infections, septicaemia, meningitis, peritonitis, gonorrhoea. Surgical prophylaxis.
Dosage/Direction for Use
Tab Usual duration: 7 days (range: 5-10 days). Adult & adolescent >13 yr Most infections 250 mg bd. Mild to moderate lower resp tract infection eg, bronchitis 250 mg bd. More severe lower resp tract infections eg, pneumonia 500 mg bd. UTI 125 mg bd. Pyelonephritis 250 mg bd. Gonorrhoea 1 g as single dose. Childn Usual dose: 125 mg bd. Acute otitis media Childn >2 yr 250 mg bd. IM/IV inj Adult 0.75 g 8 hrly. Severe, complicated infections 1.5 g 6-8 hrly. Bacterial meningitis Max: 3 g 8 hrly. Uncomplicated gonorrhoea 1.5 g IM as single dose at 2 separate sites w/ oral probenecid 1 g. Surgical prophylaxis 1.5 g pre-op & 0.75 g IV/IM 8 hrly. Prophylaxis in open heart surgery 1.5 g IV at anaesth induction & 12 hrly thereafter for total: 6 g. Infant & childn >3 mth 50-100 mg/kg daily in divided doses 6-8 hrly. More severe infection 100 mg/kg daily. Bacterial meningitis Initially 200-240 mg/kg daily IV in divided doses 6-8 hrly.
Administration
Should be taken with food.
Contraindications
Hypersensitivity to cephalosporins.
Special Precautions
Hypersensitivity to β-lactams. History of GI disease especially colitis. Pseudomembranous colitis. Prolonged use. Renal impairment. Pregnancy & lactation.
Adverse Reactions
Headache, dizziness, GI disturbances, erythema multiforme, SJS, TEN, allergic reactions, rash, urticaria, pruritus, hematologic & hepatic effects, transient pain at inj site, thrombophlebitis, fever & anaphylaxis.
Drug Interactions
Anticoagulants, heparin or thrombolytic agents. NSAIDs, salicylates or sulfapyrazone, potent diuretics. Decreased renal tubular excretion by probenecid. Antacids.
