Indications/Uses
X-ray contrast medium for cardio-, conventional cerebral & abdominal angiography, peripheral arteriography, urography, venography, CT-enhancement; lumbar, thoracic & cervical myelography.
Dosage/Direction for Use
Intra-arterial Adult Selective cerebral arteriography 270/320 mg I/mL, 5-10 mL/inj. Aortography 270/320 mg I/mL, 40-60 mL/inj. Peripheral arteriography 270/320 mg I/mL, 30-60 mL/inj. Selective visceral arteriography 270 mg I/mL, 10-40 mL/inj. Left ventricle & aortic root inj cardioangiography 320 mg I/mL, 30-60 mL/inj. Selective coronary arteriography 320 mg I/mL, 4-8 mL/inj Childn 320 mg I/mL depending on age, wt & pathology. Max: 10 mL/kg. IV Urography Adult 270/320 mg I/mL, 40-80 mL. Childn >7 kg 270/320 mg I/mL, 2-3 mL/kg, <7 kg 270/320 mg I/mL, 2-4 mL/kg. Doses depending on age, wt & pathology. Max: 50 mL. Venography 270 mg I/mL, 50-150 mL/leg. CT enhancements Adult Head CT 270/320 mg I/mL, 50-150 mL. Body CT 270/320 mg I/mL, 75-150 mL. Childn Head & body CT 270/320 mg I/mL, 2-3 mL/kg up to 50 mL or 150 mL in few cases. Intrathecal Lumbar, thoracic & cervical myelography 270 mg I/mL, 10-12 mL or 320 mg I/mL, 10 mL.
Contraindications
Hypersensitivity. Manifested thyrotoxicosis.
Special Precautions
History of allergy, asthma; may provoke anaphylactoid reactions or other manifestations of hypersensitivity. Risk of procedure-related thrombosis & embolism. Patients w/ serious cardiac disease & pulmonary HTN; acute cerebral pathology; tumours or history of epilepsy; multiple myeloma; pre-existing DM; paraproteinemias (myelomatosis & Waldenstrom's macroglobulinemia); phaeochromocytoma undergoing interventional procedures; hyperthyroidism; multinodular goiter; low seizure threshold. May aggravate symptoms of myasthenia gravis. Alcoholics or drug addicts. Extravasation. Measure serum creatinine levels in metformin-treated diabetic patients prior to therapy. Ensure adequate hydration during therapy. May affect ability to drive & use machines. Avoid additional kidney strain eg, nephrotoxic drugs, oral cholecystographic agents, arterial clamping, renal arterial angioplasty or major surgery. Concomitant use w/ β-blockers. Hepatic & renal impairment. Not to be used during pregnancy. Discontinue lactation until at least 24 hr after last dose. Young infants & neonates <1 yr; possibility of inducing transient hypothyroidism in premature infants. Elderly.
Adverse Reactions
Acute renal failure, anaphylactic or anaphylactoid shock, hypersensitivity reaction, Kounis' syndrome, cardiac or cardioresp arrest, MI.
Drug Interactions
Transient renal impairment, precipitated lactic acidosis w/ metformin. Increased risk of delayed reactions eg, flu-like symptoms or skin reactions w/ interleukin-2 given <2 wk prior to therapy. Interfered w/ thyroid function; lab tests for bilirubin, proteins or inorganic substances eg, Fe, Cu, Ca & phosphate.
