Azithromycin dihydrate
Indications/Uses
Lower resp tract infections including bronchitis & pneumonia; skin & soft tissue infections; otitis media; URTI including sinusitis & pharyngitis/tonsilitis. Uncomplicated genital infections due to Chlamydia trachomatis & non-multiresistant Neisseria gonorrhea. Alone or in combination w/ rifabutin for prophylaxis against Mycobacterium avium-intracellulare complex (MAC) infection.
Dosage/Direction for Use
Adult & elderly STD caused by C trachomatis or susceptible N gonorrhoea 1,000 mg as single dose. Prophylaxis against MAC infections 1,200 mg once wkly. All other indications 1,500 mg given as 500 mg daily for 3 days or for 5 days given as 500 mg on day 1, then 250 mg daily on days 2-5. Childn >45 kg Total dose: 30 mg/kg given as single daily dose of 10 mg/kg daily for 3 days or for 5 days given as 10 mg/kg on day 1, then 5 mg/kg on days 2-5. Acute otitis media 30 mg/kg as single dose or 10 mg/kg daily for 3 days. Ped Strep pharyngitis 10 or 20 mg/kg as single dose for 3 days. Max: Not to exceed 500 mg daily.
Administration
Should be taken with food.
Contraindications
Hypersensitivity to azithromycin or any macrolide antibiotics. Co-administration w/ ergot derivatives.
Special Precautions
Discontinue use if severe acute hypersensitivity reactions occur. Congenital or documented QT prolongation; electrolyte disturbance, clinically relevant bradycardia, cardiac arrhythmia or insufficiency. Superinfection. lnfantile hypertrophic pyloric stenosis. Concomitant use w/ drugs prolonging QT interval eg, class IA & III antiarrhythmics, antipsychotics, antidepressants, fluoroquinolones. Severe renal impairment. Not to be used in hepatic impairment. Pregnancy & lactation. Elderly.
Adverse Reactions
Moniliasis, vaginitis; thrombocytopenia; anaphylaxis; anorexia; psychiatric disorders; dizziness, convulsions, headache, somnolence, paresthesia, hyperactivity, syncope; vertigo, hearing impairment; palpitations, arrhythmias; hypotension; nausea, vomiting, diarrhoea, other GI disorders; abnormal liver function; allergic reactions; arthralgia; interstitial nephritis, acute renal failure; asthenia, fatigue, malaise.
Drug Interactions
Impaired metabolism of digoxin. Ergotism w/ ergot derivatives. Increased theopylline levels. Increased conc of phosphorylated zidovudine. Monitor cyclosporin levels. Antacids, terfenadine, coumarin-type oral anticoagulants.
