Azithromycin
Indications/Uses
Lower resp tract infections eg, bronchitis & pneumonia, odontostomatological, skin & soft tissue infections, acute otitis media; URTI eg, sinusitis, pharyngitis/tonsillitis. Uncomplicated genital infection due to Chlamydia trachomatis & non-multiresistant Neisseria gonorrhoea & chancroid due to Haemophilus ducreyi. Prophylaxis against Mycobacterium avium-intracellulare complex (MAC) alone or in combination w/ rifabutin. In combination w/ ethambutol for disseminated MAC (DMAC).
Dosage/Direction for Use
Adult including elderly STD caused by Chlamydia trachomatis, Haemophilus ducreyi or susceptible Neisseria gonorrhoea 1,000 mg as single oral dose. Prophylaxis against MAC infections HIV 1,200 mg once/wk. DMAC infections Advanced HIV 600 mg once daily in combination w/ antimycobacterial agents eg, ethambutol at approved dose. Other indications Total dose: 1,500 mg given as 500 mg daily for 3 days. Childn >45 kg & elderly Same as adult dose.
Administration
Should be taken on an empty stomach: Take at least 1 hr before or 2 hr after food.
Contraindications
Hypersensitivity to azithromycin or any macrolide antibiotics.
Special Precautions
Hypersensitivity; anaphylaxis, severe cutaneous ARs (SCARs) including SJS, TEN, DRESS & acute generalized exanthematous pustulosis. Congenital or documented QT interval prolongation, electrolyte disturbances, clinically relevant bradycardia, cardiac arrhythmia or insufficiency. Observe for signs of superinfection w/ non-susceptible organisms eg, fungi. Not to be co-administered w/ ergot derivatives. Concurrent use w/ QT interval prolonging substances eg, class IA & III antiarrhythmics, antipsychotics, antidepressants, fluoroquinolones. Significant hepatic disease. Severe renal impairment. Pregnancy & lactation. Infantile hypertrophic pyloric stenosis. Elderly.
Adverse Reactions
GI disturbances, mild neutropenia, abnormal liver function, hearing impairment. HIV patients: Diarrhea, abdominal pain, nausea, loose stools, flatulence, vomiting, dyspepsia, rash, pruritus, arthralgia. Asthenia; SCARs including SJS, TEN, DRESS & acute generalized exanthematous pustulosis.
Drug Interactions
Impaired metabolism of digoxin. Little effect on plasma pharmacokinetics or urinary excretion of zidovudine. Possibility of ergotism w/ ergot derivatives. Monitor prothrombin time w/ coumarin-type oral anticoagulants; cyclosporin levels. Neutropenia was observed w/ rifabutin. Possible interaction w/ terfenadine.
