Contents
Per mL Brinzolamide 10 mg, timolol 5 mg
Indications/Uses
Decreased IOP in adult patients w/ open-angle glaucoma or ocular HTN for whom monotherapy provides insufficient IOP reduction.
Dosage/Direction for Use
1 drop into conjunctival sac of affected eye(s) bd.
Contraindications
Hypersensitivity to brinzolamide, timolol, other β-blockers, sulphonamides. Reactive airway disease including bronchial asthma or history of bronchial asthma, severe COPD. Sinus bradycardia, sick sinus syndrome, SA block, 2nd- or 3rd-degree AV block, overt cardiac failure or cardiogenic shock. Severe allergic rhinitis. Hyperchloraemic acidosis. Severe renal impairment.
Special Precautions
Discontinue use if signs of serious reactions or hypersensitivity occur. History of atopy or severe anaphylactic reaction to variety of allergens. Not recommended in narrow-angle glaucoma. Patients w/ pseudoexfoliative or pigmentary glaucoma, compromised corneas (eg, DM or corneal dystrophies); low endothelial cell count; CV diseases (eg, CHD, Prinzmetal's angina, cardiac failure), hypotension, 1st-degree heart block; severe peripheral circulatory disturbance/disorders; resp reactions including death due to bronchospasm in patients w/ asthma, mild/moderate COPD. Hyperchloraemic acidosis; hyperthyroidism; spontaneous hypoglycaemia or labile diabetes; muscle weakness consistent w/ certain myasthenic symptoms eg, diplopia, ptosis, generalized weakness; choroidal detachment. May cause eye irritation due to benzalkonium Cl. Remove contact lens prior to application & wait 15 min before reinsertion. Administer at least 5 min apart if >1 topical ophth medicinal product is used. Concomitant use w/ surgical anaesth. Not recommended to use 2 local β-blockers or carbonic anhydrase inhibitors. May affect ability to drive & use machines. Renal & severe hepatic impairment. Not to be used during pregnancy. Lactation. Not recommended in childn <18 yr.
Adverse Reactions
Dysgeusia, punctate keratitis, blurred vision, eye pain & irritation, decreased heart rate.
Drug Interactions
Acid-base disturbances & additive effect w/ oral carbonic anhydrase inhibitors. Inhibited metabolism by CYP3A4 inhibitors eg, ketoconazole, itraconazole, clotrimazole, ritonavir, troleandomycin. Hypotension &/or marked bradycardia w/ oral Ca-channel blockers, β-blockers, antiarrythmics (eg, amiodarone), digitalis glycosides, parasympathomimetics or guanethidine. Decreased response to adrenaline. Potentiated hypertensive reaction to sudden clonidine w/drawal. Potentiated systemic β-blockade w/ CYP2D6 inhibitors eg, quinidine, fluoxetine, paroxetine. Increased hypoglycaemic effect of antidiabetic agents. Mydriasis w/ adrenaline.
