Ipratropium Br
Indications/Uses
Bronchodilator for maintenance treatment of bronchospasm associated w/ COPD including chronic bronchitis & emphysema. Concomitant use w/ inhaled β-agonists for acute bronchospasm associated w/ COPD including chronic bronchitis & asthma.
Dosage/Direction for Use
Nebuliser soln Adult including elderly & adolescent >12 yr 40 drops (0.5 mg) tds-qds. Childn 6-12 yr 20 drops (0.25 mg) tds-qds, <6 yr 8-20 drops (0.1-0.25 mg) tds-qds. Inhalation soln Adult including elderly & adolescent >12 yr 40 drops (0.5 mg), dose may be repeated until patient is stable. Childn 6-12 yr 20 drops (0.25 mg), dose may be repeated until patient is stable, <6 yr 8-20 drops (0.1-0.25 mg), dose may be repeated until patient is stable.
Contraindications
Hypersensitivity to ipratropium Br, atropine or its derivatives.
Special Precautions
Hypersensitivity. Discontinue use if paradoxical bronchospam occurs. Patients predisposed to narrow-angle glaucoma; pre-existing urinary outflow tract obstruction eg, prostatic hyperplasia, bladder-neck obstruction; cystic fibrosis. Avoid contact w/ eyes. May affect ability to drive & use machines. Pregnancy & lactation. Nebuliser soln: Asthmatic patients. May cause wheezing & breathing difficulties due to benzalkonium Cl. Not to be used concurrently w/ disodium cromoglycate.
Adverse Reactions
Headache, throat irritation, cough, dry mouth, GI motility disorders (eg, constipation, diarrhoea, vomiting), nausea, dizziness.
Drug Interactions
Intensified bronchodilatory effect by β-adrenergics & xanthine prep. Increased risk of acute glaucoma in patients w/ history of narrow-angle glaucoma w/ β-mimetics. Not recommended w/ other anticholinergic drugs.
