Donepezil HCl
Indications/Uses
Mild, moderate & severe Alzheimer's dementia.
Dosage/Direction for Use
Adult & elderly Mild to moderate Alzheimer's disease 5 mg once daily at bedtime, may be increased to max: 10 mg once daily following 1 mth of clinical assessment. Severe Alzheimer's disease 10 mg once daily.
Administration
May be taken with or without food: Take at bedtime. Place tab on the tongue & allow to disintegrate before swallowing w/ or w/o water.
Contraindications
Hypersensitivity to donepezil HCl or piperidine derivatives.
Special Precautions
NMS. Sick sinus syndrome or other supraventricular cardiac conduction conditions eg, SA or AV block; syncope or seizures; bladder outflow obstruction. Preexisting or history of QTc prolongation; Torsade de pointes; preexisting cardiac disease eg, uncompensated heart failure, recent MI, bradyarrhythmias; electrolyte disturbances eg, hypokalaemia, hypomagnesaemia. History of ulcer disease, asthma or obstructive pulmonary disease. May exacerbate/induce extrapyramidal symptoms. Concomitant use w/ succinylcholine-type anaesth; drugs affecting QTc interval. Concurrent use w/ NSAIDs. Avoid use w/ inhibitors of acetylcholinesterase agonists or cholinergic system antagonists. May affect ability to drive & use machines. Severe hepatic impairment. Not to be used during pregnancy & lactation. Not recommended in childn.
Adverse Reactions
Diarrhoea, nausea; headache. Common colds; anorexia; hallucinations, agitation, aggressive behaviour, abnormal dreams & nightmares; syncope, dizziness, insomnia; vomiting, abdominal disturbance; rash, pruritus; muscle cramps; urinary incontinence; fatigue, pain; accidents.
Drug Interactions
Inhibited metabolism by ketoconazole, quinidine, itraconazole, erythromycin, fluoxetine. Reduced levels by rifampicin, phenytoin, carbamazepine, alcohol. Interfered w/ medications having anticholinergic activity. Synergistic activity w/ succinylcholine, other neuromuscular blockers or cholinergic agonists, β-blockers. Combination w/ medicinal products prolonging QTc interval eg, class IA & III antiarrhythmics, certain antidepressants (eg, citalopram, escitalopram, amitriptyline), other antipsychotics (eg, phenothiazine derivatives, sertindole, pimozide, ziprasidone), certain antibiotics (eg, clarithromycin, erythromycin, levofloxacin, moxifloxacin).
