Protamine sulphate
Indications/Uses
Counteract anticoagulant effects of heparin pre-op; after renal dialysis & open heart surgery; if excessive bleeding occurs; when overdose has inadvertently been given.
Dosage/Direction for Use
Individualized dosage. Adult No >50 mg slow IV inj over 10 min. Unfractioned heparin neutralisation 1 mg for at least 100 IU mucous heparin or 80 u lung heparin, may be reduced if >15 min elapsed since IV inj. LMWH neutralisation 1 mg/100 u.
Special Precautions
Allergic reaction to protamine, fish. Not suitable for reversing effects of oral anticoagulants. Patients previously undergone coronary angioplasty or cardiopulmonary bypass, diabetics treated w/ protamine insulin, men who have had vasectomy or are infertile; Ab to protamine. Avoid too rapid administration. Carefully monitor clotting parameters during prolonged procedures using repeated protamine doses. Pregnancy & lactation. Not recommended in childn.
Adverse Reactions
Anticoagulant effect; hypersensitivity reactions; bradycardia; sudden BP fall, pulmonary & systemic HTN, transitory flushing, feeling of warmth; severe, acute pulmonary vasoconstriction w/ CV collapse; dyspnoea; nausea, vomiting; back pain; lassitude.
Drug Interactions
Incompatibility w/ certain antibiotics eg, several cephalosporins & penicillin. Precipitation w/ some iodinated contrast media eg, ioxaglate meglumine & ioxaglate Na-containing soln.
