CapAdult & adolescent >14 yr 50 mg, if pain relief is unsatisfactory take another 50 mg after 30-60 min. Max: Not to exceed 400 mg daily. InjAdult & adolescent ≥12 yr 1-2 amp by IV inj/IV infusion/IM/SC. If pain relief is not obtained w/in 30-60 min, may give 2nd dose of 50 mg. Severe pain 2 amp as initial dose. Max: Not to exceed 400 mg daily.

Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Resp depression, acute alcoholism, head injuries & increased ICP conditions. Not to be used in patients receiving MAOIs. Not to be used during pregnancy & lactation. Adolescents 12-18 yr who are obese & w/ obstructive sleep apnea or severe lung disease. Childn <18 yr after tonsils &/or adenoids removal.
Resp depression; decreased resp reserves, hypoxia, hypercapnia; adrenal insufficiency; shock. Concomitant use w/ anesth, naloxone, benzodiazepines, serotonergic drugs, alcohol. Long-term use. May affect ability to drive & operate machines. Renal or hepatic impairment. Not recommended during pregnancy & lactation. Not recommended in childn <12 yr. Infants especially neonates. Cap: Not to be used for minor pain. Not to be given during bronchial asthma attack or in heart failure secondary to chronic lung disease, hypothyroidism, adrenocortical insufficiency, prostatic hypertrophy, inflammatory or obstructive bowel disorders, myasthenia gravis. Inj: Hypersensitivity to opiates. Not recommended as substitute in opioid dependence; reduced consciousness disorders of uncertain origin; increased ICP due to head injuries or brain diseases. Drug abuse or dependence.
